Description Provision For The Supply Of Petroleum, Oil, Lubricants Including Servicing Of Cpd Xi Motor Vehicles And Motorcycle For Fy-2024. 1. The Supplier Shall Allow Cpd Xi To Acquire Petroleum, Oil & Lubricants On Credit. 2. The Supplier Shall Allow Cpd Xi Vehicle To Be Maintained For Change Oil, Fuel Filter And Needed Supplies For Servicing On A Scheduled Basis For Each Vehicle. 3. Withdrawal Of Petroleum, Oil And Lubricants Shall Be Through A Withdrawal Slip And Job Order Slip For Servicing Of Vehicles Duly Signed By The Ao I-gs Unit And Ao V-head Of Admin. Section Or Their Duly Authorized Representatives. 4. The Supplier Shall Issue A Delivery Receipt/credit Invoice Upon Withdrawal With Corresponding Quantity And Unit Price Per Withdrawal. 5. The Supplier Shall Give A Billing Statement Every 15th/30th Of The Month Or Upon The Request Of Cpd Xi. 6. The Cost Of Petroleum, Oil And Lubricants Shall Be Based On The Current Market Price (pump) Upon Withdrawal And Must Be Indicated In The Billing Statement. 7. The Supplier Must Comply With The Following Documents Requirements: Mayor/business Permit, Philge3ps Registration, Dti Certification, Bir 2303, Business Income Tax Return, Omnibus Sworn Statement. 8. Discount And Other Features Offered By The Supplier Will Be The Basis For Award Which Is Advantageous To The Government. 9. Location Of Supplier Must Be Within 500km Radius From Cpd Xi 10. The Supplier Must Have A Lbp Account For Issuance Of Lddap-ada Payment From Cpd Xi Subject For Bir Withholding Tax For Every Billing Statement. 11. A Contract Of Service And Possible Addendum For Additional Amount Of Contract Shall Be Executed To Effect The Abovementioned Conditions.
