Name Of Project : Cooperative Month Celebration 2024 Project Description : Cooperative Month Is Celebrated In The Entire Country For The Whole Month Of October To Recognize And Acknowledge The Significant Contributions Of Cooperatives To The Economy Of The Country Pursuant To Ra No. 11502. Project Cost : P 2,500,000.00 Lgufund : P 2,000,000.00 Ngo/po Equity : P 500,000.00 Components Of The Project: A.1. Institutional Events These Are The Events That Are Directly Relevant To The Cooperative Month Celebration. Common In All Cooperative Month Celebration Press Presentation And Community Launching Grand Opening Ceremony Closing Ceremony Trade Fair And Exhibit A.2 Variety Showcase Competitions Showcasing Talents And Skills Relevant To The Celebration, And Non-competition Events Staged And Managed Entirely By The Organizations Or By Major Sponsors; A.3capability Building These Are Activities That Will Provide Primary Cooperative Learning Opportunities Through Trainings, Seminars And Forum. 2. Manage Funds Provided For The Projects In Accordance With Philippine Accounting Standards And Coa Circulars 2007-001; 3. Generate Additional Funds For The Projects Through Rent, Fees Or Solicitation From Sponsoring Companies, Organizations Or Individuals; Submit Corresponding Reports To The City Government As Stipulated In The Moa. The Ngo/po Shall Submit A Proposal Or Application For Funding Addressed To The Chairperson – Bids And Awards Committee – Good And Consulting Services Accompanied By The Following Documents: 1. Certificate Of Registration To Cooperative Development Authority (cda); 2. Authenticated Copy Of The Latest Articles And By-laws Of Cooperative/ Articles Of Cooperation Showing The Incorporators/ Organizers; 3. Audited Fs For The Past 3 Years; If Less Than 3 Years, Financial Reports For The Years Of Operation And Proof Of The Previous Implementation Of Similar Projects; 4. Disclosure Of Other Business If Any; 5. Work And Financial Plan (wfp) Sources And Details Of Proponent’s Equity Participation In The Project Equivalent To 20% Of The Total Project Cost, Which Shall Be In Form Of Labor, Land For The Project Site, Facilities, Equipment And The Like, To Be Used In The Project; 6. Complete Project Proposal Duly Approved/ Signed By Its Officers; 7. List And/or Photographs Of Similar Projects Previously Completed Indicating The Source Of Funds For Implementation, If Any; 8. Sworn Affidavit Of The Secretary Of The Organization Signifying That None Of Its Incorporators, Organizers Or Officials Is An Agent Of Related Up To The Fourth (4th) Civil Degree Of Consanguinity Or Affinity To The Officials Of The Local Government Unit Authorized To Process And/or Approved The Proposal, Moa And Release Of Fund; 9. Valid And Current Mayor’s Permit From The Ngo’s Principal Place Of Business; 10. Certificate Of Philgeps Registration; 11. Tax Payer’s Identification Number; 12. Latest Tax Returns And Tax Clearance Pursuant To Executive Order No. 398, S. Of 2005; 13. Statement Of All Its Ongoing And Completed Government And Private Contracts Within The Relevant Period, Where Applicable, Including Contracts Awarded But Not Yet Started, If Any In Accordance With Section 23.1 (a) (iv) Of The Irr Of R.a. 9184; 14.
