Notarized Special Power Of Attorney, Notarized Secretary’s Certificate Or Board/partnership Resolution For Partnership, Corporation, Cooperative, Or Joint Venture, Whichever Is Applicable. 5. Proof Of Being In The Business Of And Operating As A Pest Control Company For At Least Three (3) Years. 6. Valid License To Operate Issued By The Food And Drug Administration (fda). 7. Valid Certificate Of Product Registration Issues By The Fda For Its Household Pesticides To Be Used For The Pest Control Activities.
