License To Operate From Fda With List Of Sources (whether It Is A Manufacturer, Importer, Seller, Or Distributor) F. Delivery Receipt / Prescribing Invoice Bearing Lot Nos. Suppliers Who Have Previously Submitted The Above Legal Requirements To The Provincial Procurement Management Division (ppmd) May No Longer Require Its Re – Submission. 4.
