Description The Al-amanah Islamic Investment Bank Of The Philippines, Through Its Bids And Awards Committee, Is Inviting All Prospective Suppliers To Submit Their Proposals For The Supply, Delivery, And Tailoring Services Of Office Uniforms For Fifty-three (53) Employees With An Approved Budget For The Contract In The Amount Of Six Hundred Two Thousand Pesos (php 602,000.00), Inclusive Of All Government Taxes And Charges. A. Tailoring Services - The Completion Period Of The Supply, Delivery, And Tailoring Services Shall Be 60 Calendar Days From Receipt Of The Notice To Proceed (ntp). This Is Inclusive Of The Conduct Of Preliminary Activities Such As, Sourcing Of Fabric Materials For The Uniforms And Taking Of Body Measurements. B. Delivery/distribution Of Uniforms - The Schedule Of Delivery, Which May Be Made On A Per Batch Basis, Must Be Coordinated With And Approved By The Human Resource Management Department (hrmd) At Least Three (3) Working Days Prior To Scheduled Delivery. - Delivery Of Uniforms Shall Be Made In Complete, Individually Packed Sets Per Employee. - The Deliveries Must Be Supported By An Appropriate Delivery Receipt(s), Which Shall Indicate The Printed Name And Signature Of Employee Or Assigned Receiving Staff As Proof Of Delivery. C. Alterations/repairs - The Supplier Must Provide Representative/s Who Will Regularly Visit The Aaiibp To Cater To The Uniform Alteration Needs Of Employees. This Must Be Conducted At Least Once A Week Beginning On The Delivery Of 1st Batch Of Uniforms, But Not To Exceed Thirty (30) Calendar Days From The Delivery Of The Last Batch Of Uniforms. All Prospective Suppliers/service Providers Shall Be Evaluated Based On Their Legal, Technical, And Financial Capacity To Deliver And Complete The Required Goods/services. Notarized Omnibus Sworn Statement 4. Current Tax Clearance Certificate (tcc) 5. Certificate Of Satisfactory Performance/acceptance From Any Of Its Previous Clients In The Last Five (5) Years 6.
