Provision of community-based support for mental health services-In 2019, Somerset ICS was awarded “Trailblazer” funding to support a transformative approach to Mental Health Services in Community Settings, with Somerset Foundation Trust being commissioned by the Commissioners to deliver these services in alignment with the principles jointly developed as part of the submission. Somerset Foundation Trust is now seeking a Provider/Alliance of Providers with whom to deliver and strengthen the service model.
