The Agency Must Have A Base Radio Communication With Duly Licensed And Authorized By The The National Telecommunication Commission. 2. The Security Personnel That Shall Be Assigned In The Dedvmh Must Have Undergone A Recent Drug Test In Accredited Institution. The Guard To Be Hired Preferably Non Smokers. 3. The Agency Must Conduct Orientation And Training To Its Security Personnel Bi-annually. 4. The Agency Must Conduct Survey And Submit Hospital Security Management Plan And Program And Security Assessment On Disaster (natural-man Made), Violence And Terror Activities. 5. The Agency Must Provide The Following Items Necessary For Security Operations: A. Emergency Flashlight For Each Security Guard On Duty B. (19pcs) Raincoats, (19pcs) Rain Boots And (7 Pcs) Umbrella For On Duty Guards C. Magnetic Metal Detectors (3pcs) D. Two(2) Megaphones E. Firearms With Complete Number Of Ammunitions F. One (1) Unit Cellular Phone For Security Communications Post In Guard House With Available Load. G. One (1) Rechargeable Emergency Lamp On The Station H. One (1) First Aid Kit For Each Security Guard On Duty I. Provide Logbooks 6. The Agency Shall Provide Additional Number Of Licensed Security Guard With Corresponding Number Of Firearms And Ammunitions Whenever A Need For Such Supplemental Security Services Arises Without The Necessity Of Executing Another Contract Agreement. Compensation For Said Additional Forces Si In Accordance With The Contract Agreement 's Provision On Compensation. The Bidder Shall Also Assure A 24-hour Security Including Saturday, Sunday And Holidays. The Medical Center Chief Or Its Duly Authorized Officials Has The Right To Assign Of The Guards In Different Designated Areas. 7. The Guards Are Mandated To Protect Lives, Secure Properties From Theft, Robbery, Pilferage And Prevent All Unlawful Acts In The Premises Of The Client. 8. The Security Guards Supplied By The Agency Under This Contract Shall In No Case Be Considered As Employees Of The Client. 9. The Agency Must Provide Relievers Or Replacements In Case Of Absences Of Any Of The Assigned Security Guards. 10. The Agency Must Provide 24 Hours A Day, Seven Days A Week A Supervisor/security Officer To Be Assigned In The Hospital To Exercise Close Supervision To The Security Guards To Ensure Faithful, Continuous And Uninterrupted Service. 11. The Agency Shall Hold Responsible Or Liable For Any Loss, Injury Or Damage Sustained Or Caused By Negligence Of The Security Guards Of The Agency During The Performance Of Their Duties. The Cost Of Such Losses, Repair Of Damages And Treatment Of Injury Shall Be Charged To The Agency. 12. The Client, If It So Desires And For Reasons Of Its Own, May Order The Agency For Suspension/replacement Or Any Or All Security Guards Assigned By The Latter And That The Decision In This Regard Is Final. 13.the Agency Shall Faithfully Comply With The Statutory Remittances Such As Sss, Pag-ibig And Philhealth. As Proof Of Compliance, It Shall Furnish Client A Copy Of The Receipt Of Premiums Of Monthly Basis. 14. The Agency Shall Provide The Following Extra Service: A. Intelligence And Investigation Services (overt/covert) B. Periodic Assessment On Security Measures C. Day And Night Spot Inspections By Agency Supervisors/inspectors D. Attend And Assist Security Matter Or Concerns E. Request Military And Police Assistance As Needed F. Provide Extra Guards Free Of Charge During Special Events Or Occasions 15. The Agency, Must Be Compliant With Department Order No. 174 Of The Department Of Labor And Employment (certificate Of Registration From Dole) 16. The Security Guards Must Strictly Maintain Security And Safeguard The Buildings, Officers And Properties Within The Premises Of Don Emilio Del Valle Memorial Hospital As Well As To Provide Protection For Its Officials, Employees, Visitors, Guest And Transacting Public. 17. The Security Agency Shall Require Its Security Guards To Wear Prescribed Uniform While On Duty. Security Agency Shall Provide Logbook In Order To Properly Maintain A Record Of Attendance Of The Employees In And Out Record On Visitor's Vehicles, Watchers And Patients. 18. The Security Guard Must Strictly Imposed The Strictly No Smoking Policy With In 100 Meters Radius Outside Hospital Premises. 19. The Security Guard Must Follow The Visiting Hours' Policy In The Hospital. 20. The Security Guard Must Ensure The Safety Of Dedvmh Parking Area To List All Visitors' Vehicle (number Of Hours Allowed 8 Hours Only) And Employees Vehicle. 21. Monitor The Aircon Schedule Of On/off In All Offices And Other Private Rooms And Must Be Recorded In Logbooks. 22. Shall Report And Coordinate To Dedvmh Engineering Department And Abide Hospital's Other Work Related Policies/rules. 23. The Procuring Entity Will Conduct Inspection Anytime If All Abovementioned Technical Specifications Were Delivered. 24.
