Details: Description I. Scope Of Work: The Work Consists Of The Furnishing Of All Necessary Labor, Materials, Tools And Equipment, Including Technical Supervision Necessary For The Complete Execution Relative To The Repair Of Visitor’s Vehicle Lane Area At Pnoc Main Entrance Gate Are The Following: A. General Requirements 1. Mobilization A. Site Temporary Enclosure Using Blue Sack Or Any Suitable Materials That May Enclose The Workplace. 2. Construction Of Project Billboard In Compliance To Coa Circular No. 2013-004 3. Construction Health And Safety A. Proper Hauling Of Materials. B. Personal Protective Equipment C. Installation Of Safety Signage And Cautions. 4. Proper Disposal Of The Debris Outside The Energy Compound Or On A Dumpsite. 5. Submission Of As- Built Plans 6. Conduct A Field Density Test And Other Testing Of Materials. 7. Demobilization Includes Cleaning Up Of Site, Clearing, Hauling And Disposal Of Waste And Construction Debris. Restoration Of Any Damages Shall Also Be Done Before Exiting The Area. B. Earthworks 1. Conduct Survey, Layout, Stakeout, And Removal Of Existing Structure Or Obstruction For Site Preparation. C. Sub Grade Preparation 1. Use A Compactor For Correction And Compaction Grade Before Laying Gravel Fill. Secure A Maximum Dry Density Compaction Test Of 95%. 2. Gravel Fill Must Be At Least 75mm Thickness D. Concrete Pavement 1. Provide Necessary Formworks 2. Driveway And Parking Area Shall Be 200mm Thick And Adopt 3,500 Psi At 28 Days Concrete Strength. Use A Ready-mix Concrete. 3. Provide 20mm Dia. Deformed Dowel Bars Spaced At 300mm. 4. Provide Contraction, Expansion/ Construction Joints For Concrete Pavement. 5. The Contractor Shall Submit Copy Of Manufacturer’s Certificate Of Mill Tests Of All Reinforcing Steel. 6. Compression And Slump Tests Shall Be Made For Every Batch Of Concrete. Submit The Result Of All Tests. Ii. Labor Requirements: Contractor Must Be Knowledgeable In General Construction And Horizontal Projects. The Minimum Key Personnel And Work Experience Requirements Are The Following: Key Personnel Relevant Experience One (1) Project Engineer Minimum 3 Years Of Experience As A Project Engineer In General Construction. One (1) Safety Officer Minimum 2 Years Of Experience As A Safety Officer In General Construction. One (1) Foreman Minimum Of 3 Years Of Experience As A Foreman In General Construction. The Contractor’s Project Engineer, Safety Officer, And Foreman Must Always Be Present On Site Whenever Work Is Being Performed. Iii. Equipment Requirements: The Minimum Major Equipment Requirements Are The Following: Equipment Capacity Number Of Units Jackhammer - At Least One (1) Air Compressor - At Least One (1) Plate Compactor At Least 5 Hp At Least One (1) Bar Cutter - At Least One (1) Concrete Vibrator - At Least Two (1) Iv. Material Requirements: 1. Sub Grade Preparation - Base Course Should Consist Of Granular Fill Materials. - Gravel Fill Should Consist Of ¾” Materials. 2. Reinforcing Steel Bar - Bar Reinforcement For Concrete Structures, Bars Shall Be Deformed In Accordance With Aashto M 31. - Use 20mm Deformed Reinforcing Bars Grade 40, 300mm Spacing O.c For Dowels. 3. Concrete Pavement A. Portland Cement Concrete Pavement - Pccp Should Be In 3,500 Psi Compressive Strength B. Joint Sealer To Be Applied In All Pccp’s Construction Joints Must Be Rubberized Liquid Asphalt And Should Be A Weather- Proof Type Of Sealing Compound. V. Bill Of Quantities Please Refer To Section Viii. Vi. Project Execution Requirements A. Methodology Requirements 1. Submission Of Gantt Chart For Determination Of Activities And Timelines. 2. Mobilization A. The Contractor Shall Validate And Check Conditions On-site Which May Have An Effect On The Work And Shall Immediately Notify Pnoc Before Commencing Works. B. A Coa Project Billboard Shall Be Posted On The Construction Site On A Tarpaulin Sign Board Suitably Framed For Outdoor Display At The Project Location And Shall Be Posted As Soon As The Award Has Been Made. The Tarpaulin Shall Have The Following Specifications: Tarpaulin, White 8ft X 8ft Resolution: 70 Dpi Font: Helvetica Font Size: Main Information –3” Sub-information – 1” Font Color: Black C. The Contractor Shall Provide Safety Signages/ Early Warning Signs That Are Visible At The Jobsite. D. The Contractor Shall Install, Operate And Maintain Adequate Number Of Temporary Hoists, Scaffolds, Runways, Ladders, And The Line As Required For The Proper Execution Of The Work. Safety Precautions Shall Be Observed At All Times. E. All Temporary Services And Facilities Installed By The Contractor Shall Be Removed By The Contractor Upon Completion Of This Contract. The Contractor Shall Restore Damaged, And/or Altered Areas Caused By Such Removal And During Project Implementation. 3. Conduct Survey, Layout, Stakeout, And Removal Of Existing Structure Or Obstruction For Site Preparation. 4. The Contractor Shall Submit Detailed Engineering Drawings And Layouts For The Parking Lot And Visitor Vehicle Lane For The Approval Of The Pnoc Representative. 5. The Contractor Shall Submit A Concrete Pouring Request And Coordinate The Pnoc Representative For The Inspection Of The Area Prior To The Concreting Works. 6. The Contractor Shall Submit Initial As-built Plans For Comments And Approval Of Pnoc Representative. 7. The Contractor Shall Submit Three (3) Copies Of Signed And Sealed As-built Plans In A3 Size, Along With Cad And Pdf Files For The Digital Copy. 8. The Contractor Shall Submit All The Results Of Material Testing And Project Completion Report With Completed Signatories. 9. Hauling Of All Debris Outside The Pnoc Energy Compound. Additional Requirements: A. General Conditions 1. The Contractor Must Do Inspection And Actual Measurements Of All Areas As Specified By The Scope Of Work Prior To Submission Of Bid In Order To Arrive At An Estimate Of The Labor, Materials, Equipment, Facilities And Services Necessary To Carry Out The Work. No Inspection Shall Be Ground For Disqualification Of Bid. 2. The Contractor Is Required To Secure The Required Surety, Performance And Guarantee Bonds Prior To The Commencement Of Work. 3. The Contractor Shall Commence The Work On The Agreed Start Date And Shall Carry Out The Work In Accordance With The Program Of Work Submitted By The Contractor, As Updated, With The Approval Of The Pnoc Admin-gsd Representative And Complete The Project By The Completion Date As Indicated On The Tor. 4. The Contractor Shall Request A Pre-construction Coordination Meeting Before Mobilization. 5. The Contractor Must Comply With All Standard Health And Safety Requirements To Include, Among Others, The Use Of Personal Protective Equipment By All Workers, Installation Of Safety Signages, Proper Wearing Of Id And Observe Company Rules And Regulations On Safety And Security. 6. The Work Shall Be Executed In The Best And Thorough Manner Throughout The Project Timeline And To The Satisfaction Of Pnoc-admin Gsd, As Represented By Its Engineers And Supervisors Who Will Jointly Interpret The Meaning Of The Scope Of Work And Its Conditions And Shall Have The Power To Reject Any Work, Method Of Accomplishing Every Part Of Work And Material Used Which In Their Judgement Are Not In Full Accordance With The Specifications And/or Description As Provided In The Scope Of Work And Are Deemed To Be Disadvantageous To Pnoc. 7. A Joint Inspection Must Be Conducted By The Contractor And Pnoc Representatives On The Specific Works To Be Done For The Project. 8. The Contractor Is Required To Submit The Following Items Subject For Approval Of Pnoc Prior To Commence Of The Work: A. Construction Methodology B. Product Data Sheet/ Material Specifications C. Boq With Weighted Percentage D. Manpower List E. Tools And Equipment List F. Work Schedule And S-curve; And G. Safety Program 9. The Contractor Shall Secure The Pnoc Work Permit System Prior To Execution Of Works: 10. The Contractor Must Conduct Proper Planning And Safety Orientation Meetings With All Personnel Involved In The Work Prior To Commencement. 11. The Contractor Is Required To Coordinate All Activities And Work Relative To The Project With The Pnoc Representative For Proper Monitoring. 12. The Contractor Shall Be Solely Liable For All Accident Claims And Other Related Claims Arising From Injuries And Damages, Which May Occur In Relation To The Execution Of The Project. 13. The Contractor Shall Hold Pnoc And Its Personnel Free From Any And All Liabilities For Damages Arising From Injuries Or Liabilities To Persons Or Damages To Property Occasioned By Any Act Or Omissions Of The Contractor Including Any And All Expenses Which May Be Incurred By Pnoc And Its Personnel In The Defense Of Any Claim, Action Or Suit. 14. The Cost Of Any And All Rework And/or Restoration Of Damaged Properties Due To The Contractor’s Poor Workmanship Or Negligence Shall Be Solely Borne By The Contractor. 15. The Contractor Shall Submit To Pnoc-admin Gsd Representative Daily Accomplishment Report For Construction Monitoring Purposes. 16. The Contractor Shall Be Liable For All Direct And Consequential Damages Arising Out Of Any Failure To Perform The Work In Accordance With The Schedule And With Terms And Conditions Of The Contract Documents. B. Site Possession 1. The Contractor Confirms That He Has Inspected The Site Where The Work Will Be Performed; That He Is Fully Aware Of The Conditions Which Might Affect The Nature, Extent, And Cost Of The Work Required; That Pnoc Shall In No Way Be Responsible For Any Costs Or Expenses Which May Be Incurred On Account Of The Failure Of The Contractor To Make An Accurate Examination Of Present And/or Assessment Of Future Factors That May Affect The Cost Of Execution. 2. Tapping Of Equipment And Other Related Work Shall Be Properly Coordinated With Pnoc-admin Gsd Designated Representative For Provision Of Electrical Power Source For Outlets. 3. The Gsd Shall Designate A Specific Area To Be Used By The Contractor As Temporary Facilities/storage Area/ Administration Are In Connection With The Project. C. Contract Interpretation The Contract Documents, Specifications And Drawings Are Mutually Complimentary. In Case Of Conflict, What Is Noted In One Although Not Shown In Another Shall Be Considered Contained In All. The Specifications Shall Prevail Over The Drawings. The Documents Forming The Contract Shall Be Interpreted In The Following Order Of Priority: 1. Contract/construction Agreement 2. Letter Of Acceptance 3. Contractor’s Bid 4. Specifications 5. Drawings 6. Bill Of Quantities 7. The Contractor Shall Examine All The Issued Documents Relating To The Work And Must Have Verified All Existing Conditions At The Site. No Consideration Will Be Given For Any Alleged Misunderstanding Or Misinterpretation Of The Specifications And Materials To Be Used Or Work To Be Done. All Works Shall Be Properly Coordinated With Pnoc- Admin Gsd. Iv. Project Duration Work Should Be Completed Within Forty-five (45) Calendar Days Reckoned From The Date Of Receipt Of The Notice To Proceed. Work Schedule: 7:00 Am Onwards On Weekdays, Weekends And Holidays. Note: Delays Due To Work Stoppage (related To Contractor’s Noncompliance To Safety, Health, Security, And Environmental Policies Of Pnoc-admin Gsd) Ordered By Pnoc-admin Gsd Will Still Be Counted To The Original Contract Duration; I.e. No Suspension Of Time Shall Be Granted To The Contractor. V. Warranty Provisions The Warranty Will Cover Two (2) Years For Workmanship, Starting From The Date Of Final Turnover And Acceptance. Any Corrective Work Required Will Be Performed At No Additional Cost To The Owner. Vi. Definition Of Similar Projects The Contractor Must Have Completed At Least One (1) Project In Horizontal Civil Works. The Contractor Must Be A Small B Contractor With Pcab License In Category C Or D.

Laws, Ordinances , Rules And Regulations 1.21 In Submitting A Proposal/bid ,the Bidder Is Considered To Have Familiarized Himself, Acts, Rules And Regulations Of The Republic Of The Philippines, Including Any Or All Local Ordinances Or Rules Or Regulations, Which In Any Manner May Affect Or Apply To The Operation And Activities Of The Bidders. The Bidder Shall Be The Employer Of The Personnel Whom He Will Engage The Prosecution Of The Contract And Is Under Obligation To Comply With The Labor Laws Of The Philippines. 1.22 In Addition , The Bidder Is Advised To Carefully Examine And To Inform And To Familiarized Himself With All The Ordinances ,rules, And Regulations Adopted By The Barangay In Regards With The Procurement Of Goods. Bidders Competence 1.23 The Barangay Reserves The Right To Fully Examine The Competence And Responsibility Of The Bidder At Any Time Before Award Of Contract By Aby Means And To Reject Any Bid/tender When Statement As To Business And Technical Organizations Or Financial Resources Or Construction Performance Have Been Found To Be False /and Or Fabricated And In The Barangay’s Opinion Would Justify Rejection. Pre Bid Conference 1.24 No Pre Bid Conference Will Be Conducted Except Upon Written Request Of Any Prospective Bidder Which Shall Be Held Within A Reasonable Period Before Prospective To Adequately Prepare The Bids Subject To The Approval Of The Bac. 1.25 Bids Will Be Opened On November 12, 2024 At Exactly 2:00 Pm At The Barangay Hall. Bidders Or Authorized Representative May Attend The Opening Of Bids. 1.26 Immediately After Opening Of Bids The Bac Of The Barangay Will Undertake A Thorough Study And Bid Evaluation Of Proposed Submitted. Award Of The Contract Shall Be Made To The Bidder With The Lowest Calculated Responsive Bid. 1.27 The Barangay Reserves The Right To Reject Any Or All Bids /tenders ,to Waive Informality In The Bid/tender Received .the Right Is Also Reserve To Reject The Bid/tender Of Any Bidder Who (a)have Previously Failed To Satisfactorily Perform Or Complete Any Construction Contract Undertaken By Him. (b)was Qualified In The Eligibility Requirement On The Basis Of Suppressed Or False Information; Or (c) Such Other Compelling Grounds As Determined During Bid Evaluation Of The Proposal Of Bids; 1.28 Within A Period Not Exceeding Fifteen (15) Calendar Days From The Declaration By The Bac Of The Lowest Calculated Responsive Bid Or Highest Rated Responsive Bid And Immediately After Approval Of The Recommendation Of The Bac By The Local Chief Executive Of His Duly Authorized Representative Shall Issue The Notice Of Award To The Winning Bidder. Withdrawal Of Bids 1.17 A Bidder May Modify Its Bid Before Deadline For The Submission And Receipt Of Bids Not Allowed To Retrieve Its Original Bid, But Shall Send Another Bid Equally Sealed, Properly Identified And Marked As A Modification And Stamped Received By Bac. 1.18 A Bidder Will Be Allowed To Withdraw His Bid Prior To The Time Set For Its Opening Provided He Communicated His Purpose In Writing To The Bac; In This Case The Bid/tender Shall Return To Him Un Opened .no Bid Can Be Withdrawn For Any Reason Whatsoever After Opening Of Bids Has Commenced. 1.19 The Bidder Shall Be Responsible For Having Taken Steps To Carefully Examine All The Contract Documents ,have Fully Informed Himself With All Conditions, Local Or Otherwise ,relative To The Carrying Out Of The Contract Work; 1.20 The Barangay Shall Not Assume Any Responsibility Regarding Erroneous Interpretations Or Conclusions Obtained By The Bidder Out Of The Data Furnished By The Barangay Or Its Consultant, Either Before Or Conditions Contained In The Contract. Laws, Ordinances , Rules And Regulations 1.21 In Submitting A Proposal/bid ,the Bidder Is Considered To Have Familiarized Himself, Acts, Rules And Regulations Of The Republic Of The Philippines, Including Any Or All Local Ordinances Or Rules Or Regulations, Which In Any Manner May Affect Or Apply To The Operation And Activities Of The Bidders. The Bidder Shall Be The Employer Of The Personnel Whom He Will Engage The Prosecution Of The Contract And Is Under Obligation To Comply With The Labor Laws Of The Philippines. 1.22 In Addition , The Bidder Is Advised To Carefully Examine And To Inform And To Familiarized Himself With All The Ordinances ,rules, And Regulations Adopted By The Barangay In Regards With The Procurement Of Goods. Bidders Or Authorized Representative May Attend The Opening Of Bids. 1.26 Immediately After Opening Of Bids The Bac Of The Barangay Will Undertake A Thorough Study And Bid Evaluation Of Proposed Submitted. Award Of The Contract Shall Be Made To The Bidder With The Lowest Calculated Responsive Bid. 1.27 The Barangay Reserves The Right To Reject Any Or All Bids /tenders ,to Waive Informality In The Bid/tender Received .the Right Is Also Reserve To Reject The Bid/tender Of Any Bidder Who (a)have Previously Failed To Satisfactorily Perform Or Complete Any Construction Contract Undertaken By Him. (b)was Qualified In The Eligibility Requirement On The Basis Of Suppressed Or False Information; Or (c) Such Other Compelling Grounds As Determined During Bid Evaluation Of The Proposal Of Bids; 1.28 Within A Period Not Exceeding Fifteen (15) Calendar Days From The Declaration By The Bac Of The Lowest Calculated Responsive Bid Or Highest Rated Responsive Bid And Immediately After Approval Of The Recommendation Of The Bac By The Local Chief Executive Of His Duly Authorized Representative Shall Issue The Notice Of Award To The Winning Bidder.
