Procurement Notice Construction Works Modernization of the Multifunctional Playground at Primary School No. 16

1. The subject of the Order is the execution of designs and obtaining the necessary decisions, opinions and permits, including the decision on the building permit or the notification of the execution of construction works, and the performance of construction works in the field of construction of sports facilities in the Cybinka commune (in the Design and Build mode). 2. As part of the Implementation of the Subject of the Agreement (implementation of the above-mentioned Investment Task), the following Activities are envisaged: Construction of a full-size grass pitch in the town of Białków on the plot with Geodetic No. Cybinka - Commune, Construction of a sports field on the plot with Geodetic No. 44, Registration Precinct 145, Registration Unit Cybinka - City, in the town of Cybinka, at ul. Dąbrowskiego 32. 3. The scope of the Contractor's services includes the preparation of design documentation and obtaining the necessary decisions, opinions and permits, including the decision on the building permit or notification of the execution of construction works, and the performance of construction works in the field of construction of sports facilities in the Cybinka commune, including M. Others: 1) Development of the Design Documentation in accordance with the Functional-Utility Program, hereinafter referred to as Pfu (Appendix No. 6 to the Swz) containing: A) Preparation of a Map for Design Purposes covering the Investment Area - 1 copy - If Required, B) Obtaining On behalf of and for the benefit of the Employer, a decision on determining the location of a public purpose investment - if required, C) development of a construction design in accordance with the Construction Law, the Regulation of the Minister of Development of September 11, 2020 on the detailed scope and form of the construction design, and in accordance with Regulation of the Minister of Development and Technology of December 20, 2021 on the detailed scope and form of the design documentation, technical specifications for the execution and acceptance of construction works and the Functional and Utility Program - in paper form - 4 copies; D) Development of executive designs, in accordance with the Regulation of the Minister of Development and Technology of December 20, 2021 on the detailed scope and form of design documentation, technical specifications for the execution and acceptance of construction works, and the functional and utility program, taking into account possible protections or removal of conflicts with the existing network of technical infrastructure With the Planned Land Development - 4 copies; E) Development of detailed technical specifications for the execution and acceptance of works (hereinafter Stwiorb) in accordance with the Regulation of the Minister of Development of September 11, 2020 on the detailed scope and form of the construction design, and in accordance with the Regulation of the Minister of Development and Technology of December 20, 2021 On the detailed scope and form of the Design Documentation, Technical Specifications for the execution and acceptance of construction works and the Functional-Utility Program - 2 copies; F) Preparation of the bill of quantities in accordance with the Regulation of the Minister of Development and Technology of December 20, 2021 on determining the methods and grounds for preparing the investor's cost estimate, calculating the planned costs of design works and the planned costs of construction works specified in the Functional and Utility Program - in paper form ( 1 Copies); G) Development of Information on Safety and Health Protection (bioz) taking into account all industries - 2 copies; M) Obtaining on behalf of the Employer the legally required decisions, opinions, arrangements and verifications of the Design Documentation for the Planned Project; N) Obtaining the Employer's approval by submitting an application for a construction permit or notification of the execution of construction works, For building; Whereas the entirety of the above-mentioned The documentation should also be submitted in digital/electronic form (on a CD/DVD, all in *.pdf files and editable versions) - 1 copy. of the following Tasks: A) Task No. 1: "Construction of a Full-Size Grass Field in the City of Białków" Including the Construction of a Sports Field With a Natural Grass Surface (on Plot No. 4/18, Registration Precinct 1-Białków, Registry Unit Cybinka - Commune) with the dimensions of the playing field - 68.00 MX 105.00 M, execution of the irrigation system for the pitch, delivery and assembly of street furniture (litter bins, shelter for players, goals), delivery and assembly of tribunes for fans, assembly of barrier netting , Installation of Two Wickets. B) Task No. 2: "Construction of a sports field in Cybinka" Including the construction of a multifunctional sports field with a surface made of synthetic grass along with equipment (volleyball and football field) in place of one of the existing sports fields with a grass surface (on the Plot Geodetic No. 44, Registration Precinct 145, Registration Unit Cybinka - City, in the town of Cybinka, at 32 Dąbrowskiego Street) About the dimensions of the playing field of the pitch with artificial grass surface - 27.00 MX 45.00 M, drainage of the pitch, delivery I Assembly of complete pitch equipment, Lighting of the designed pitch, Installation of barrier nettings around the entire multifunctional pitch, Installation of one wicket netting netting in the fencing of the pitch, Delivery and assembly of small architecture (litter bins, park benches, Shelter for players). 3) Preparation of as-built documentation in accordance with the requirements of the Construction Law, along with the provision of surveying services; Including the Geodetic As-built Inventory - in paper form (1 copy) and in electronic form on a CD/DVD (1 copy); 4) Obtaining, on behalf of the Employer, all the necessary decisions enabling the use of the completed investment. 4. The Design Documentation should be developed in accordance with the requirements set out in the PFU, the provisions of law and the arrangements resulting from the terms of the contract between the Employer and the Contractor. Each part of the documentation must have a statement by the designer and verifying that it is made in accordance with applicable Polish regulations and design guidelines and is complete from the point of view of the purpose it is to serve. The Design Documentation must have the Contractor's statement about the verification, agreement and readiness for its implementation. 5. The detailed scope of the Subject of the Order (detailed terms and conditions of the Agreement and its scope, including the scope of design works and the scope of works to be performed) is defined by the Functional and Utility Program (hereinafter referred to as Pfu) constituting Appendix No. 6 to the Swz (respectively for each Task) and Draft Agreement constituting Appendix No. 5 to the Swz.
