Buckinghamshire Council Is Seeking To Commission An Organisation That Will Act As A Single Point Of Contact To Provide Pre- And Post-diagnostic Advice And Support For Individuals And Their Carer's Throughout The Dementia Journey. The Service Will Empower People To Make Informed Decisions About Their Care To Maximise Their Quality Of Life, Reduce The Risk Of Crisis And Admission To Hospital And Enable The Individual To Live Independently In Their Communities For As Long As Possible. The Service Will Also Offer Training To A Range Of Health And Social Care Professionals To Identify Common Early Symptoms Of Dementia, Complete Memory Impairment Screening, And Direct Individuals To The Dementia Diagnostic Pathway. A Budget Cap Of £356,000 Per Annum Has Been Set For The Provision Of Dementia Support Services. The Total Value Is £1,068,000 For The Full Contract Term (including Extension). The Contract Dates Will Be 1st April 2025 To 31st March 2027 (one Optional Extension 1st April 2027 To 31st March 2028. The Council Is Of The Opinion That Tupe Will Apply.
