Details: Description Terms Of Reference Semi-annual Preventive Maintenance, Repair And Annual Calibration Of Steam Sterilizer Hi Temperature And Low Temperature, Washer Disinfector And Ultrasonic Washer. ( January To December 2025)- One Year Contract A. Scope Of Work 1. Semi- Annual Preventive Maintenance Of The Unit. 2. Detailed Inspection Of Ageing Parts And Cleaning Of The Ff. Parts - Pipe Line - Steam Generator, Vaporizer And Heater - Chamber - Softener And Filtration System - Valves And Restrictors - Vacuum And Generator Pumps - Electronic Board And Electrical Cabinets - Covers And Utility Boxes 3. Verification Of Temperature And Pressure Values/data With Calibration Certificate. 4. Validation Of Result/output Product After Sterilization Or Disinfection To Assure Output By Means Of Chemical Or Biomedical Indicators. 5. Supply And Installation Of All Ro Water Filters. B. Requirements 1. The Service Engineer/technician Must Be Trained With Certificate To Conduct Service Maintenance From The Manufacturer On Our Existing Equipment. 2. The Service Provider Must Submit An Original Copy Of Preventive Maintenance Report And Service Report. 3. The Service Provider Agrees To Provide At Its Own Expense Cleaning Materials, All Necessary Grease And Lubricants Necessary For The Maintenance Of The Unit. 4. The Service Provider Shall Perform Verification Testing/ Calibration Of All The Unit And Must Be Submitted A Copy Of Calibration Report With Certificate To Biomed Office And End-user. C. Repair Services: 1. Repair Services Is For Situations Where The Steam Sterilizer, Washer Disinfection Of This Contract Either Breaks Down Completely Or Has A Malfunction And Is Not Operating As It Should And Thus Requires An Additional Unplanned Service Visit Including The Software Update By The Service Provider’s Technician At Lpgh & Stc Request. 2. Service Provider Shall Immediately Respond And Conduct Inspection Within Twenty-four (24) Hours From The Receipt Of Report From Lpgh & Stc In Case Of Any Abnormal Operation, Breakdown, And/or Stoppage Of The Steam Sterilizer, Washer Disinfector, And Ultrasonic Washer. 3. Upon Inspection, The Service Provider Shall Conduct The Necessary Repairs To Ensure That Safety And Functionality Of The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer. In The Event The Service Provider Assesses That The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer Requires The Replacement Of Parts, It Shall Give Lpgh & Stc Its Recommendation For Replacement Of Parts And A Price Quotation From Its Suppliers Of The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer Parts Necessary For Replacement. Recommendation For Replacement Of Parts And Price Quotation From Suppliers Must Be Approved By Lpgh & Stc. All Parts Supplied By The Service Provider Shall Be Original Parts Or Similar Functionality And Quality As Original Parts. 4. In Case Of Unavailability Of Parts From The Service Provider’s Supplier For Whatever Reason Which Shall Cause Delay, Lpgh & Stc Shall Have The Option To Purchase The Necessary Parts From Another Source Through The Applicable Mode Of Procurement Allowed Under The Law In Order To Avoid Unnecessary Delay In The Works. 5. Upon Arrival Of The Parts, The Service Provider Shall Ensure That The Parts Are Properly Installed And That The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer Shall Fully Function Upon The Parts Installation. In Case, The Third-party Supplier For Parts Purchased By Lpgh & Stc Under Paragraph Iv Caused The Installation Of The Parts As Required By Its Warranty, The Service Provider Shall Ensure That The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer Parts Are Properly Installed In Compliance With Its Recommendation And Report. D. Call Back Services. 1. Call Back Services Is For Situations When Within The Ten (10) Days After Preventive Maintenance Or Repair Service Works, There Is A Stoppage Or Malfunctioning Of The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer. 2. The Service Provider Shall Immediately Respond Within Twenty- Four (24) Hours From The Receipt Of Report From Lpgh & Stc In Case Of Any Abnormal Operation And Stoppage Of The Steam Sterilizer, Washer Disinfector And Ultrasonic Washer Within Ten (10) Days After Preventive Maintenance. 3. The Service Provider Shall Provide Call- Back Services At No Charge. Call Back And Repair Service Coverage Does Not Cover The Cost Of Materials And Labor For Any Additional Repairs Required To Bring The Equipment Back Into Operation. E. Penalty 1. If The Service Provider Failed To Complete The Preventive Maintenance Of All The Unit And Have An “unsatisfactory” Rating On Our Evaluation Form For Two (2) Consecutive Months, The Contract May Be Terminated And They May Be Blacklisted. F. Warranties 1. The Service Provider Guarantees That The Equipment Will Function Properly And Runs Smoothly For Twenty-four (24) Hours For Ten (10) Days Continuous Running Time Immediately After A Repair Or Preventive Maintenance. 2. The Service Provider Guarantees That The Service Rendered Will Avoid Frequent Breakdown Of The Equipment Or Its Operations. The Service Provider Further Warrants That All Works Covered In This Contract Are Done To The Best Of Their Abilities And Shall Ensure The Proper Functioning Of The Covered Equipment. Any Repairs Or Problems Incurred Within Ten (10) Days From The Date Of The Last Work Or Preventive Maintenance Shall Not Be Subjected To Any Additional Costs On The Client.
