Notes for serial item No 01 to 03:- (i) The details of maintenance to be carried out during daily /fortnightly /monthly /quarterly / half yearly /yearly. Log sheet of the maintenance will be submitted by the contractor on monthly basis duly signed by service engineer and contractor. Format of logsheet will be approved by Engineer-in-charge. (ii) The rate quoted is inclusive of the cost of all kind of checking and for earth resistance test. (iii) The rate quoted is inclusive of repairs /replacing of earthing system as required on ground. (iv) The rate quoted is inclusive of the cost of components generally changed due to wear and tear and not considered under Sch A part III, such as parts of electronic control panel's power contactor, break shoe liner, cables for inter connections, springs, bearings, bushing, gibs, fuses, nuts and bolts etc., (v) The visit of executives/ experts be intimated well in advance so that periodical maintenance can be supervised, schedule of periodical maintenance (daily, fortnightly, monthly, quarterly, half yearly and annually) be submitted well in advance for approval by Engr-in-charge. (vi) The AMC should cover any/ all parts of lifts if its functions found defective the same should be repaired or replaced by the OEM as per the timeline covered under norms. (vii) The contractor has to carry out the AMC of the lifts through OEM only. (viii) All maintenance should be carried out in presence of concerned JE E&M and OEM Authorised Lift engineer maintenance slip will be submitted accordingly. (ix) The rate quoted is inclusive of annual inspection of lift by PWD authorised lift inspector and the report should be submmited to GE office accordingly.

SCHEDULE 'A' PART-II (LEFT TECHNICIAN) Providing Lift Technician for passenger lift of capacity 10 passengers (680 Kgs) and 27 passengers (2000 Kgs) - 02 Nos of Make : M/s KONE, installed at Renu building from 0600 hours to 1400 hours and 1400 hours to 2200 hours in two shifts of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lift of capacity 10 Passengers (680 Kgs) and 27 Passengers (2000 Kg) of Make : M/s KONE, installed at Renu building from 2200 hours to 0600 hours in One shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lift of capacity 8 passengers (884 Kgs) of Make : M/s 'Otis', installed at Type-II building (A Block) Chunabhatti from 0600 hours to 1400 hours and 1400 hours to 2200 hours in two shifts of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Providing Lift Technician for passenger lift of capacity 8 passengers (884 Kgs) of Make : M/s 'Otis', installed at Type-II building (A Block) Chunabhatti from 2200 hours to 0600 hours in One shift of 08 hours each per day, including Sunday and holidays all as per latest applicable lift rules complete all as specified and as directed by Engr-in-charge.

Notes for serial item No 4 to 7 :- (i) The lift rescuer (operator) shall be Matriculate with sufficient experience (not less than 05 years). The lift rescuer (operator) should be capable to rectify both mechanical as well as electrical defects for smooth functioning of lifts. It is responsibility of contractor to provide a reliever to the rescuer (operator). So that lift rescuer (operator) is available all time during operation period for uninterrupted functioning of lift. (ii) Man power list with aadhar linked bank account number, qualification certificate will be submitted prior to commencement of Rescue Operation for necessary sanction from GE. (iii) The contractor/ service providing agency shall be wholly and exclusively responsible for payment of wages of persons engaged by it in compliance with all the statutory obligations under all related legislations as applicable to it from time to time including Minimum Wages Act, ESI Act and the Deptt. MES shall not incur any liability for any expenditure whatsoever on the persons employed by the contractor/ Agency on account of any obligation. The contractor/ service providing agency will be required to provide particulars of Group Insurance of its Employees engaged by him to the GE before processing of RAR payment. (iv) The contractor/ service providing agency will be required to provide two sets of uniform to all operators with name and firm tag.

SCHEDULE 'A' PART-III (ESSENTIAL REPAIRS) Supply and fix in repairs main hoist rope multistranded size 10 mm dia heavy duty confirming to ISI standards complete all as specified and as directed by Engr-in-charge.

Supply and fix in repairs governor rope of dia 6 mm multistranded of ISI make and pullies of suitable size complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement Car door operator drive compact inverter with door motor complete set for door opening and closing operation system suitable for 'Kone' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement Car door operator drive compact inverter with door motor complete set for door opening and closing operation system suitable for 'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Break coil for break operation system (Set of two coils) suitable for 'Kone' and 'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Landing door header with mechanical and Electrical accessories suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of ARD PCB Card (Input and output card for ARD Operation) suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Shift LOPB card for Landing and Calling operation suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in repairs existing infra-red door protection edge/ sensor (Lambada) with latest new infra red door protection edge for full height of car door edge for 10 to 27 passengers lift to serve G+6 floors suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Material and labour in replacement of electric hooter 230 V, 50 Hz loud siren type complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Landing door seal for 13 Passengers lift as per existing make of lift complete all as specified and as directed by Engr-in-charge.

Material and labour in replacement of Cabin fan suitable for 10/27 passengers lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Main mother board (Logic board) in main panel board after power supply card and giving command to relay card for 10/27 passengers lift suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix car door operating rope made out of steel strands, 4 mm thick having 25 strands thimble and length adjustable screw approx. 1.5 metre length suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Replacements of complete hall push button having button for call registration and LED dot matrix indicators having stainless steel 2.0 mm thick face plates including micro processor card suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Landing door belt suitable for 'Kone'/'Otis' make lift complete all as specified and as directed by Engr-in-charge.

Supply and fix in repair rechargeable, maintenance free lead acid batteries of 12 Volt, 17 AH for ARD system complete all as specified and as directed by Engr-in-charge.

Supply and fix in replacement of Lift car door seal as per existing make of lift 'Kone'/'Otis' make 10 to 27 Passengers lift complete all as specified and as directed by Engr-in-charge.
