Description Bids And Awards Committee __________________________ __________________________ __________________________ Request For Proposal / Invitation For Negotiated Procurement – Small Value Procurement For The Tarpaulin Printing Services For Fy 2024 (pr No. 2024-01-018) The Northwest Samar State University (nwssu) Through Its Bids And Awards Committee (bac) Hereby Invites Interested Suppliers To Submit Proposals For The Tarpaulin Printing Services For The University For Fy 2024, Duly Signed By Authorized Representative Not Later Than January 24, 2024 @ 9:00 Am Subject To The Terms And Conditions Provided In This Rfp. Terms And Conditions: 1. The Supplier Shall Allow Nwssu To Provide Tarpaulin Printing Services On Credit Not Exceeding To The Total Approved Budget For The Contract Of One Hundred Thousand Pesos (php100,000.00); With Layout: By Square Foot (width X Length) @ Php_________ No Layout: By Square Foot (width X Length) @ Php__________ 2. Maximum Size Of Tarpaulin Printing Offered Width:_________ Length:_________ 3. The Contract Shall Be For A Period Of Twelve (12) Months, Or Until Such Contract Price Is Consumed, Whichever Comes First, To Commence Upon The Approval Of The Contract; 4. The Supplier Shall Accommodate The End-user’s Request For Tarpaulin Printing Services Upon The Approval Of The Contract Agreement Duly Signed By The University President Or His Duly Authorized Representative And Approved Request Slip By The Vice President For Administrative Affairs Or His Duly Authorized Representative; 5. The Supplier Shall Deliver The Item/s And Issue A Delivery Receipt With Corresponding Quantity And Amount Per Tarpaulin Printing Services To The Supply Office For Inspection And Acceptance Of Said Item/s; 6. The Supplier Shall Issue A Billing Statement Every 5th Of The Succeeding Month And Submit To The Procurement Office; Payment Shall Be Made In Calbayog City; 7. The Supplier Must Have A Landbank Account And Comply With The Following Documentary Requirements: A. Mayor’s/business Permit B. Philgeps Registration No./certificate C. Original Copy Of Omnibus Sworn Statement 8.
