As Such, Suppliers Of Unknown Qualifications Are Hereby Invited To Submit Their Quotation/proposals As Supply Section For The Item/s Described, Subject To The Terms And Conditions Provided Below. A. Copies Of Eligibility Requirements, (valid Mayors’/business Permit Or Licenses B. Tax Identification No. (tin) C. Philgeps Registration Along With The Oath Inclusion D. Quotation/proposal E. Omnibus Sworn Statement The Income/business Tax Return Shall Also Be Required For Submission By The Bidder, Having The Lowest Calculated Bid Before Notification Of Award.
