Description Request For Quotation Doc Control No. : Form-as-pd-002 Revision No: 0 Effectivity Date : 1 September 2023 The Department Of Human Settlements And Urban Development (dhsud) Would Like To Invite Your Company To Submit A Quotation Or Proposal For The Following Procurement Activity: Request For Quotation No. Rfq No. 04-0165, S. 2024 Procurement Activity Supply Of Labor, Tools, Materials And Technical Supervision For The Pest Control And Disinfection Services For Dhsud Central Office Building (1st Rebidding) Mode Of Procurement Negotiated Procurement – Small Value Procurement (section 53.9) Approved Budget For The Contract (abc) One Hundred Thousand Two Hundred Sixty-nine Pesos And 00/100 (php100,269.00), Inclusive Of All Existing And Appropriate Government Taxes And Charges. Deadline For The Receipt Of Quotations Or Bid Proposal On Or Before 1:00 Pm, 8 April, 2024. Delivery Schedule Delivery Of Goods/items Shall Be Seven (7) To Twenty (20) Calendar Days Upon Receipt Of The Approved Purchase Order. The Minimum Technical Specifications For The Said Activities Are As Follows: Terms Of Reference Supply Of Labor, Tools, Materials And Technical Supervision For The Monthly Pest Control And Disinfection Services Of The Dhsud Central Office Building For Cy 2024. I. Background/rationale The Dhsud Occupies As Its Central Office A 9th Story Building Located At Kalayaan Avenue, Corner Mayaman Street, Diliman Quezon City. This Project Covers General Pest Control And Disinfection Maintenance Services Of The Dhsud Offices And Its Premises To Eradicate The Infestation Of Pests/insects And Kill Bacteria/viruses In Order To Provide A Healthy And Conducive Work Environment To All Officials And Employees, As Well As To The People And Stakeholders Who Will Transact Business With The Department. Ii. Contract Period The Contract Period Is For Nine (9) Months From April To December 2024 With The Delivery Of Services To Start From The Approval Of The Purchase Order. Iii. Approved Budget For The Contract (abc) The Abc For The Project Is One Hundred Thousand Two Hundred Sixty-nine Pesos And 00/100 (php100,269.00) Inclusive Of All Applicable Government Taxes As Included In The Dhsud Approved Annual Procurement Plan For Fy 2024. Iv. Scope Of Work The Service Provider Shall Provide The Department With The Following Services: 1. The Service Provider Shall Provide All Labor, Materials, Tools, Equipment, Supervision, And Other Incidentals For The General Pest Control And Disinfection Of The Dhsud Building. 2. Provision Of At Least Nine (9) Monthly Comprehensive Treatments Of Pest Control And Disinfection Services For The Contract Duration; 3. Conduct Of Thorough Survey And Inspection Of Dhsud Offices And Its Premises To Pinpoint Pest Infestations And Installation Of Control Mechanisms For Pest Exclusion And Prevention; 4. Conduct Of Monthly Monitoring And Assessment Of The Department’s Offices And Premises For Signs Of Infestation And Submit A Report Thereof To The Director Of The Administrative Service; And 5. Sanitizing With Disinfectant All Areas To Kill Any Viruses And Bacteria. . V. Duties And Responsibilities Of The Service Provider The Service Provider Shall: • Conduct Of A Comprehensive Pest Control And Disinfection Treatment Services Every Last Sunday Of The Month For All Dhsud Offices And Premises Such As, But Not Limited To Residual Insecticidal Spraying, Misting, Larviciding, And Disinfection; • Regular Treatment By Bait Laying Of Anticoagulant Preparations In Areas Used As Passageways And Harborages Of Rats And Mice, Based On A System Of Permanent/semi-permanent Bait Stations. Supplementary Control Methods Such As Mechanical Traps And Sticky Boards, Plugging/sealing Off Of Holes That May Serve As Entry Points Will Also Be Implemented If Needed; • Cracks, Crevices And Hidden Surfaces Shall Also Be Treated With Insecticidal Solutions. Residual Misting And /or Spraying Shall Be Applied To Areas Where Fogging Treatment Is Not Advisable; • Ensure That All Labor, Materials/chemicals And Other Supplies Needed For The Delivery Of Services Will Effectively Terminate Any And All Pests, Insects, Rodents, Viruses, And Bacteria; • Conduct Of Monthly Monitoring And Assessment Of The Dhsud Offices And Its Premises To Assess The Effectiveness Of Treatment Used And Recommend Changes, If Any Without Cost To The Department; • Use Only Chemicals That Are Duly Approved By The Food And Drug Administration (fda) And Fertilizers And Pest Authority Of The Philippines (fpap) In The Pest And Disinfection Treatments; Submit A Service Treatment Report To Gsd Personnel After Every Treatment Conducted Indicating Therein The Areas That Were Treated And Monitored, Duly Confirmed/acknowledged By The Gsd Personnel Present During The Treatment. Vi. Terms Of Payment The Service Provider Shall Be Paid On A Monthly Basis Upon Inspection And Acceptance By General Service Division And Duly Noted By The Undersecretary For The Administrative And Finance Service, Subject To The Submission Of A Monthly Billing Invoice And Service Treatment Report. Vii. Qualifications Of The Service Provider The Service Provider Shall Submit The Following Documents: 1. Latest Business Permit Showing That Its Primary Business Is Pest Control And Disinfection Services And With At Least Five (5) Years Of Experience In The Business; 2. Philgeps Registration Number; 3 .latest Certificate Of Accreditation From The Food And Drug Administration (fda) Of Accredited Pest Control Organization; 4. Latest Training Certificate Of Pest Control Service Technicians From The Fda Accredited Training/facility Organization; And; 5. Certificate Of Product Registration Of Pesticides To Be Used In The Dhsud From The Fda/fpap And Material Data Sheet From The Distributor Or Manufacturer. Terms And Conditions: 1. Delivery Of Goods/items Shall Be Seven (7) To Twenty (20) Calendar Days Upon Receipt Of The Approved Purchase Order; 2. Payment Shall Be Made Upon Completion Of Service/delivery And Submission Of The Supporting Documents (e.g., Delivery Receipt, Sales Invoice, Billing Statement, Warranty Certificate If Applicable, And Other Documents As May Be Required In The Technical Specifications); 3. The Department Shall Have The Right To Inspect The Goods To Check Its Conformity With The Required Minimum Technical Specifications; 4.
