Proper Sealing And Marking Of Quotations- For Physical Submission Interested Bidders Are Required To Accomplish, Provide Correct And Accurate Information, And Submit One (1) Original And Additional Two (2) Certified True Copies Of Their Duly Signed Price Quotation Form (annex “a") Using Their Official Letterhead And Other Documentary Requirements As Provided In Section A. 1. And Enclose The Same In One (1) Sealed Envelope. The Sealed Envelope Shall Be Marked As To Include The Following: (1) Name And Address Ofthe Dofi - Las Pinas Drug Abuse Treatment And Rehabilitation Center Bids And Awards Committee, (2) Title And Abc Ofthe Project, (3) Rfq Number Ofthe Project, And (4) Name And Business Address Ofthe Supplier. (5) Email Address And Contact Number Ofthe Supplier The Improper Sealing And/or Marking Ofthe Quotations Shall Not Disqualify A Prospective Bidder. Flowever, The Bids And Awards Committee Shall Assume No Responsibility For The Misplacement Ofthe Contents Ofthe Improperly Sealed Or Marked Quotation. C. Amount And Denomination Of Quotation Price Quotations Exceeding The Abc Shall Be Rejected. All Price Quotations Shall Be Quoted In Philippine Peso. D. No Multiple Submission Ofquotations Multiple Submission Of Quotations Shall Not Be Allowed. If During The Evaluation Of Quotations, A Supplier Is Found To Have Submitted Two Or More Different Quotations For The Same Project, All Submitted Quotations Ofthe Prospective Supplier Shall Be Disregarded And Shall Be Returned To The Latter. E. Validity Ofquotation Price Quotations Must Be Valid For A Period Ofthirty (30) Calendar Days From The Date Ofsubmission Ofquotations. F. Alterations, Erasures And Overwriting Any Alterations, Erasures Or Overwriting Shall Be Valid Only Ifsigned Or Initialed By The Bidder Or His/her Duly Authorized Representative/s. G. Right Ofinspection The Doh-lpdatrc Shall Have The Right To Inspect And/or Test The Goods To Confirm Their Conformity With The Technical Specifications. H. Award Of Contract Award Of Contract Shall Be Made Per Lot Which Is Found To Be The Lowest, Responsive And Compliant With The Minimum Technical Specifications And Other Tenns And Conditions Specified Herein. I. Warranty Security The Supplier Shall Be Required To Put Up A Warranty Security In The Form And Amount As Allowed Under Sec. 62.1 Ofthe Irr Of R.a. No. 9184, To Wit: For The Procurement Of Goods, In Order To Assure That Manufacturing Defects Shall Be Corrected By The Supplier, A Warranty Security Shall Be Required From The Contract Awardee For A Minimum Period Ofthree (3) Months, In The Case Of Expendable Supplies, Or A Minimum Period Of One (1) Year, In The Case Of Non-expendable Supplies, After Acceptance By The Procuring Entity Of The Delivered Supplies. The Obligation For The Warranty Shall Be Covered By Either Retention Money In An Amount Equivalent To At Least One Percent (1 %) Of Every Progress Payment Or A Special Bank Guarantee Equivalent To At Least One Percent (1%) Ofthe Total Contract Price. The Said Amounts Shall Only Be Released After The Lapse Ofthe Warranty Period Or, In The Case Of Expendable Supplies, After Consumption Thereof: Provided, However, That The Supplies Delivered Are Free From Patent And Latent Defects And All The Conditions Imposed Under The Contract Have Been Fully Met. J. Liquidated Damages Liquidated Damages Equivalent To One Tenth Ofone Percent Ofthe Value Ofgoods Undelivered Within The Prescribed Delivery Period Shall Be Imposed For Every Day Ofdelay. The Doh - Lp Datrc Shall Have The Right To Rescind The Contract When The Cumulative Amount Of Liquidated Damages Reaches Ten Percent (10%) Of The Contract Price, Without Prejudice To Other Courses Of Action And Remedies Available To It. K. Reservation Clause The Doh - Lp Datrc Reserves The Right To Accept Or Reject Any Or All Quotations And To Impose Additional Terms And Conditions As It May Deem Proper.
