Project Description The Annual Physical Examination (ape) Is A Mandatory Tool To Monitor The Health Status Of Employees By Providing On-site Services Like Routine Diagnostic, Screening And Other Ancillary Procedures To Ensure Early Detection And Adequate Treatment Of Any Illnesses. B. Objectives To Ensure A Safe, Efficient And Effective Workplace For Dbplc’s Personnel And Its Clients. C. Duration Of The Project The Target Date For The Conduct Of The Annual Physical Exam Is Within February 2024. Actual Date Of Implementation Will Be Coordinated Between The Service Provider And End-user Following The Completion Of All Documentary Requirements Of This Procurement. D. Coverage A Dbplc Employee Holding A Permanent Position And Who Is In The Service As Of The Date Of Implementation Shall Be Covered. Newly-hired Employees With Pre-employment Medical Screening And Examinations Less Than A Year From The Date Of The Subject Procurement Ape Execution Shall Not Be Required To Undergo The Ape Under This Procurement. E. Scope Of Work A. The Following Required Diagnostic And Screening Procedures/examinations Shall Vary Depending On The Specific Age Bracket: (25 Employees Excluding Those Who Are Newly-hired Within 12 Months) 1. For 39 Years Of Age And Below (male And Female): (10 Employees) A. Physical Examination B. Basic Optical Examination (visual Acuity View) C. Routine Urinalysis D. Complete Blood Count (red Blood Cells, White Blood Cells, Hemoglobin, Hematocrit, Platelet Count) E. Chest X-ray (plain): Posterior-anterior (pa) View F. Blood Typing 2. For 40 Years Of Age And Above (male And Female): (11 Employees) A. Physical Examination B. Basic Optical Examination (visual Acuity View) C. Routine Urinalysis D. Complete Blood Count (red Blood Cells, White Blood Cells, Hemoglobin, Hematocrit, Platelet Count) E. Chest X-ray (plain): Posterior-anterior (pa) View F. Blood Typing G. Blood Chemistry • Fasting Blood Sugar • Lipid Profile O Cholesterol O Triglycerides O Hdl O Ldl O Vldl O Chol/hdl Ratio • Creatinine • Uric Acid • Liver Enzymes O Sgpt O Sgot • Blood Urea Nitrogen F. Technical Specifications And Other Conditions • The Diagnostic Laboratory Must Be Reputable And Duly Accredited By The Department Of Health (doh) • All Diagnostic Procedures Of Dbplc Employees Shall Be Performed At The Dbplc Office. • The Service Provider’s Clinic Location Should Be Within A 5-kilometer Radius From The Dbplc Office Located At The Pacific Star Building, Makati City. • The Ape Shall Be Conducted In Two (2) Batches, Maximum Of 2 Days. • All Diagnostic Procedures Of Dbplc Employees Shall Be Performed On-site (per Batch/schedule) 1 Representative Per Group Per Day (maximum Of 2 Days) • If The Dbplc Employee Is Not Available On Both Schedules, The Employee Will Have The Option To Go To The Service Provider’s Clinic To Avail Of The Ape Services/procedure Outside The Scheduled Date. • The Diagnostic Laboratory Should Have No Derogatory Record From Doh Or Any Of Its Attached Agencies In The Last 5 Years. • The Facilitator Must Process The Necessary Professional Certification/s Or License/s In Order To Perform The Services Procured. • The Medical And Diagnostic Laboratory Should Have A Full Service Complement To Carry-out The Annual Physical Exam On-site At The Dbplc Office And Its Clinic. • The Service Provider Shall Not Use Or Disclose Confidential Information, Or Any Part Thereof, In Any Manner Other Than Is Necessary To Perform Its Services Under This Agreement Or As Required By Law. • The Service Provider Shall Secure The Written Waiver Of Rights From The Employees Of The Right To The Confidentiality Of Information Between The Physician And Patient For Procedures And Examinations Undertaken In Behalf Of Dbplc. The Written Waiver Shall Include The Authorization For The Service Provider To Submit The Results Or Medical Information To Dbplc. All Documents/ Medical Examination Results Shall Be Filed In The Health Profile Of The Employees. G. Mode Of Procurement Small Value Procurement Under Sec. 53.9 Of The Irr Of Ra 9184 H. Source Of Fund Corporate Operating Budget 2024 I. Terms Of Payment 1. Payment Shall Be Based On The Bid Price Of The Service Provider. Bid Price Should Be Inclusive Of All Taxes And Fees Such As But Not Limited To Vat, Transportation Costs (for On-site Medical Examination). 2. The Service Provider Shall Forward The Statement Of Account (soa) And Other Pertinent Documents Especially The Consolidated Report For The Services Rendered Within Ten (10) Working Days From The Last Scheduled Testing/screening Date. 3. The Dbplc Shall Have A Period Of Fifteen (15) Working Days To Process Payment From The Day Of The Complete Submission Of All Documents And Reports From The Service Provider Namely: A. Statement Of Account; B. Procedure/examination Results C. Transmittal List Containing Name Of Employee, Date Of Examination/procedures, Types Of Procedures /examination, Results J. Others ● The Service Provider Should Coordinate With The Hr Specialist For Further Instructions. ● Interested Service Provider Is Required To Submit The Following Documents: 1. Valid And Current Mayor’s Permit/business Permit 2. Philgeps Registration Number 3. Professional Certification/s Or License/s Of The Facilitator
