Copper Tube, Circuit Breaker, Nema, Electrical Wires, Moldings, Metal Support Frame. • Other Parts/materials Need For The Installation Of Indoor And Outdoor Unit. • Excess Copper Tube Will Be Charge Per Actual Foot. Interested Suppliers Are Required To Submit Original/certified True Copy Of The Following: 1) Valid And Current Mayor’s Permit, (in Case Of Expired Mp/bp, Submit Copy Of Recently Expired Permit And Official Receipt As Proof Of Renewal); 2) Dti/sec Registration (for Partnerships/corporations, Attached General Information Sheet & Articles Of Incorporation); 3) Philgeps Registrations Certificate/registration Number; 4) Duly Signed Price Quotation Form (annex “a”); 5) Certificate Of Registration/tac Clearance Certificate; And 6) Original Signed Omnibus Sworn And Statement (oss).
