Description I. Description General Pest Control Service Of The Industry And Investments Building For A Period Of One (1) Year. Ii. Approved Budget Of The Contract The Approved Budget For The Contract Is Two Hundred Fifty Thousand Pesos (php 250,000.00) Inclusive Of Vat. Iii. Scope Of Work • Eliminate And Control All Of Crawling And Flying Insects; Rats And Mice In All Areas Of The Building From Basement To Penthouse And Parking Area. • Conduct Weekly General Pest Control Including Thermal Fogging To Be Undertaken Every Saturday Of The Month In The Following Areas: Offices, Lobby, Hallways, Stairways, Comfort Rooms, Machine/garbage Rooms, And Other Areas Of The Building To Eradicate Cockroaches, Mosquitoes And Other Flying And Crawling Insects. • Use Misting Machines With Water-based Chemicals Duly Approved By Food And Drug Administration (fda) Inside Offices, Canteen And Stock Rooms. • Control And Eradication Of All Flying And Crawling Insects By Using Surface Spray And Injection Gel. • Utilize Rodenticides Bait Capable Of Killing Rats And Mice By Strategically Placing It In Common And Other Susceptible Harborage Areas. • Install Sticky Traps, Mechanical Barriers And Automatic Multiple-capture Rat Traps In Areas Considered As Nesting Place Of Rats And Mice. • Sealing Of Potential Or Active Entry Points, Removal Or Reduction Of Possible Active Harborage Sites. • Use Of Anti-coagulant Poison To Avoid Bait Shyness And Outsmart The Sharp Instinct Of The Rats And Mice. This Shall Be Done Weekly With A Follow-up Check-up On The Second Day From The Day Of Bait Application. • Provide Additional Service If There Will Be A Sudden Outbreak Of Infestation In The Treated Areas Without Additional Costs To Boi. Note: The Bidders Shall Submit The Above Scope Of Work With Indication Of Compliance With Each Item Stated Therein. Otherwise, The Bidder Shall Be Disqualified. Iv. Liability Of Service Provider In Case Of Damage To Boi Property Or Injury To Boi Personnel The Service Provider Shall Be Liable To Pay The Actual Damage In Case Of Damage To Or Loss Of Property Including Injuries To Its Personnel During The Conduct Of Pest Control Services. V. Documents For Submission • Mayor’s Permit • Company Profile • Documents That Prove At Least Ten (10) Years Of Experience In The Business • List Of Clients For The Five (5) Years • Updated Certificate Of Registration To Dbm-ps Philgeps • Income/business Tax Return • Fda Certificate Of Product Registration • Fertilizer And Pesticide Authority (fpa) Training License Vi. Terms Of Payment Boi Shall Pay The Service Provider Within Fifteen (15) Working Days Upon Submission Of The Billing Statement/invoice. Vii. Contract Termination The Boi Reserves The Right To Terminate The Contract In Whole Or In Part In Case Of Delay And Failure To Perform Its Obligations Specified In The Service Contract. Viii. Contract Duration The Contract Shall Be Effective For A Period Of One (1) Year Contract Duration From Receipt Of The Notice To Proceed Subject To Automatic Extension/renewal For A Period Of Three (3) Months From Expiration Of The Contract Until A Prospective Bidder Has Been Declared As The Winning Bidder. Ix. Reservation Clause The Boi Reserves The Right To Reject Documents That Do Not Comply With The Requirements, Waive Any Formalities Of Documents Or Consider Any Submission Of Documents As Substantial Compliance Reject Any And All Bids; Declare A Failure Of Bidding, Annul The Bidding Process, Or Not To Award The Contract, Without Thereby Incurring Any Liability To The Affected Bidder Or Bidders Or If The Funds/allotment For The Program/project/activity Has Been Withheld Or Reduce Through No Fault Of The Boi.
