A copy of the tender notice is enclosed ( Annexure-I) 2. An Affidavit as per specimen copy is enclosed ( Annexure-II) 3. Authority letter from Principal/manufacturer (Annexure-III) 4. The detailed technical specifications/descriptions and stores quantity required are given as per Annexure A-I is attached. 5. The tender form along with Annexure I, II, III & Annexure A-I should be returned intact duly filled and signed, Infringement of this condition shall render the tender liable to rejection. 6. The sealed tender should bear the tender Inquiry No. 109 on the preface of the envelope and it should be clearly be super-scribed as “Tender for purchase of Bowie & Dick Test Pack through rate contract for the period of two years “Due on 30.10.2018 7. The tenders received without GST number and other documents called for may render the tender invalid. TERMS AND CONDITIONS 1. (a) Tenders should be sealed with cello tape. Unsealed tenders will not be entertained and such tenders will be ignored straightway. (b) Tenders must be submitted on the prescribed Tender Form otherwise tender will be ignored straightway. 2. (i) Tenders must accompanied with an Earnest Money for a sum of Rs. 15000/- (Rupees fifteen thousand only) in the name of Director, PGIMER, Chandigarh in the shape of either: (a) Deposit-at-call Receipt (b) Terms Deposit Receipt [Please tick in the appropriate box] Note: Earnest Money must be valid for one year from any of the commercial bank. ii) The Public Sector Undertaking of the Central/State Govt. and bidders who are registered with the National Small Industrial Corporation (NSIC) is exempted from furnishing Earnest Money alongwith tender. (iii) Earnest Money deposited with PGI in connection with any other tender enquiry even if for same/similar material / Stores by the tenderer will not be considered against this tender. (iv) The Institute will return Earnest Money to the unsuccessful tenderer after the finalization of the tender enquiry. (v) The Earnest Money of the successful tenderer would be returned to the tenderer on their furnishing of Performance Bank Guarantee as specified in condition No.3 below. 3. PERFORMANCE BANK GUARANTEE (i) The successful tenderers shall be required to furnish a security deposit in the shape of fixed deposit receipt/term deposit receipt from any of the commercial bank in an acceptable form for an amount of five percent of the estimated value of the contract/value of supplies to be made by them towards ensuring of supplies in time as defined in condition no. 17 and 37. The supplier must submit the Performance bank Guarantee within a period of 21 days after the placement of the supply order/award of contract, failing which the order/contract will be liable to be cancelled and the ‘Earnest Money’ forfeited. The performance bank guarantee will be released to the tenderers after completion and acceptance of the supplies and inspection of goods /successful performance of the contract or after two years from the placement of order/contract whichever is later. (ii) The supplier can give an undertaking for converting their Earnest Money into performance Bank Guarantee and send the balance amount equivalent to 5% of the total estimated value of goods in the shape as specified 2(i) above subject to increase in the validity period as required in 3(i). PBG should remain valid for the period of sixty days beyond the date of compliance of all contractual obligation of the supplier including warrantee obligation or one year from the placement of order/award of contract whichever is later.
